Existing customer - Refill Order by Fax

Step 1 Print the Refill Order Form OR, if you'd like to order a new prescription along with your refill to save on shipping costs, print the Refill With New Prescription Form.

Step 2 Printing clearly, complete the appropriate form and fax or mail it to us. If you are ordering a new prescription as well as a refill, you must also fax or mail us your new prescription. Be sure to request that your refill and new prescription be packaged together.
